Diabetes and Pre-Diabetes Management
Have you been diagnosed with diabetes or pre-diabetes? Our Family Health Team can provide you education and skills on how to self-manage your diabetes.  For our diabetic patients, our Nurses and Dietitian can help you. This program is offered to diabetic, pre-diabetic and impaired glucose tolerance patients.
Requirements:Â must be an enrolled patient at the FHT and 18 years of age or older.
