 Our Commitment to You

The Southlake Academic Family Health Team  (SAFHT) is committed to delivering the best patient-centered care and providing access to team-based primary care to all of our patients. We are determined to stand at the forefront of innovation and be leaders in healthcare for our region. In order to achieve these goals, we need to understand what is important to our patients. The Southlake Academic Family Health Team is developing a  5-year strategic plan that will act as our blueprint to provide better care for patients and their families. 
 

We want your feedback

We are looking to recruit patients with experience in the SAFHT’s programs (e.g. Heart Health, Diabetes Management, Healthy You, Smoking Cessation, Craving Change, Mind Over Mood or Personal Growth Programs) or care from our allied health team (e.g. Nurses, Nurse Practitioners, Pharmacists, Social Workers, Case Workers and Dietitian) and are willing to share their thoughts and experiences in an interactive group session.  You will also be able to vote on priorities that you feel are the most important to you. We will use your feedback to help shape our direction and vision in the coming years! 
 

Where will this take place? 

We will be holding these sessions on September 16th at the
 
Cardinal Golf Course (2740 Davis Drive West, King City, ON L7B 0G7) 
 
 

When will the Sessions Occur? 

In order to accommodate all schedules, we will be running one session in the afternoon and one in the evening. Refreshments will be provided. You can participate in one of the two available sessions: 
 

1. 2:30 PM – 4:30 PM 

2. 6:30 PM – 8:30 PM

 

Who will be leading these sessions? 

We have an external facilitator who will guide you through the session and discussion. They will take all the feedback you provided and incorporate it into our upcoming 5-year strategic plan.
 

What happens after I sign up?

After you sign up, we will email you additional information to help you get thinking about some topics and questions that we might talk about. In approximately 90 days after the session, we will email all participants with an update.
